Description des services:
|
Coordinates community services and supports for children, youth, and adults with intellectual and developmental disabilities, as well as complex needs, who live in Halton
Adults (18+):
- provides service coordination
- short term and crisis counselling
- provide assistance with budgeting and managing funds
- assistance in finding accommodation
- provide referral and assistance in accessing community resources and supportive counselling
- assist with coordination and future planning

Children (under 18):- coordinated intake for children for developmental services in Halton: initial contact point for families and individuals who are interested in accessing residential and non-residential supports
- provides assistance to families who have a child with developmental disability, for information, referral and accessing services
- assist with coordination of services and future planning
- administer funds authorized by the Special Services at Home (SSAH) Program for children * provide families and or caregivers the option to have funds paid to either the worker or the family * assist in the delivery of the Special Services at Home Program

Respite Program:- provides short-term relief to families and or caregivers who support an individual with a developmental disability including people on the autism spectrum disorder (ASD)
- provides relief both in and out of the home

PassportONE Program:
- helps adults with a developmental disability be involved in their community and live as independently as possible by providing funding for community participation services and supports, activities of daily living and person directed planning
- for adults over the age 18 years and eligibility is confirmed by the Developmental Services of Ontario (DSO)
- helps young people make the transition from school to life as an adult in the community
- supports families and caregivers of an adult with a development disability so they can continue in their supportive role
